I would choose to get killed by a weapon that has a nasty side effect of giving eternal life ;D
My second choice is dying in my sleep.

Why does everyone want to die in his sleep? That's so boring! You only get to die once, why not make it exciting?

At the end of my life, I'd want to get away from it all, and relax. A peaceful death would be good for me after the life I'd lived.

Hmm. . . Maybe I'd die on a roller coaster. I can just see it. WHOOSH! The cars fly off the rail and smash into the ground. hopefully it wouldn't traumatise too many of the screaming onlookers.
A fast, nearly painless, and exciting death! No long dreaded anticipation of pain and suffering. Plus, it would get a lot of other people to appreciate their lives more, knowing that it can be taken away at any moment.

Many, many years from now, when I'm old and grey, after the final curtain call of my final performance, to applause and cheers, I'll pass on to the pearly gates in the stage wings. Nah, I hope - as most do - to die peacfully, surrounded by loved ones; and unravaged by senility, dementia (like Alzheimer's), and with as little stress as possible for the ones I may leave behind.
