With Javascript codes, is it better to have them remotely hosted, or directly inserted into the headers or footers? Also, what are some fast, reliable JS hosts? Thanks!

It's faster if they are remotely hosted after the first download of the file. The reason is because your browser stores the file in your cache which is stored on your computer. Anytime it comes across the remotely hosted JS file in the source, it retrieves it from your cache instead of downloading it again. This also means that if you update the code you would need to do a hard refresh (CTRL F5) on the file so that it updates otherwise your computer will continue using what's on your computer. As for hosts, I use ripway but it hasn't always been that reliable to be honest. I'm just too lazy to find another and upload all of my stuff to it. Just take a look at some of the hosts in this post.

For the first download, non-remote is faster (by a few milliseconds), because it doesn't have to connect to an external server. For every download after the first download, remote is much faster because the file is most likely cache'd.
Plus, it makes it easier to navigate your headers/footers.

What's the policy regarding remotly hosting scripts that aren't yours? Obviously, I wouldn't rehost scripts that say "DO NOT REHOST", but what about other scripts? I normally try to ask permission of the creator first, but sometimes I can't get in contact with them, or they're no longer here.

If it says do not rehost, then don't rehost it unless you get permission. I've treated Crossfury differently because Shin hasn't been around in years, and I haven't been able to contact him. I honestly don't think he would be upset knowing that we had to rehost his script since his host died. Thousands of people use the code and no one is taking credit from him.

I see no problem with it since there's nothing different except the code is being read from somewhere else other than your header or footer. Just leave the copyright message with it.

I don't see why anyone would care about rehosting codes. It's not like rehosting it somehow removes any "this code created by User X" notices. Perhaps just leave the copyright notices in the external script.
e.g. <script src="EXTERNAL URL" type="etc"> // this code (C) whoever // other notices </script>
