That is the support forums current skin converted to its v5 equivalent. In your profile you will also be able to try out the other v4 support skins in v5

Try signing in with your username from here and your v5 pw. It should take you through the upgrade process.

It says to enter your username and password in the error.

Enter your username and password and it will then upgrade your account. After that you will use your email address and password from then on.
