Plugin Request - Custom Post Message Form

I need a something like this: support.proboards.com/index.cgi?board=codedatabase&action=display&n=1&thread=70752 that will display a custom form instead of the standard post message page. I would prefer it to be a plugin, and not a code, but it must be V5 compatible. I know of two codes that do this on V4, the one is on PBS and linked to above (the better one), and one on some other website that I can not find now (not as good). Thanks.

You can do this without a code.

Admin Home > Themes > Layout Templates > Posting Page

You need to be comfortable with HTML though.

If you have an idea of what you actually want, try and put an image together and post that, so then someone will have an better idea of how you want it

I am not in beta zone yet, I am very excited, and waiting for an invite. But I am not comfortable with HTML. I might be able to get it with detailed instructions and some help. I am not sure what you mean by "put an image together". Thanks.

He just means to sketch out your idea in Photoshop or Microsoft Paint and post it here so we can see what you have in mind.

Something like this. I just put the example code linked to in the first post into my V4.5 test forum. But the fields have to be modifiable. Thanks.

I know you coders are very busy, but I really need this plug-in. I have seen multiple other users request it. It is essential to my project. I am already late getting this project started because I have been waiting on V5. Out of these 150+ plug-in requests, this was the very first submitted, and I was hoping for it to be one of the first ones done. Thanks.
