Try moving the Smiley plugin to the very bottom of your plugin list. If that doesn't work, see if the plugin will work with the standard.
Something is either stopping the plugin from adding the events to the form buttons, or removing them after they have been added.

OK.. next step is to temporarily disable the Yootil plugin. This will stop all of the plugins that use the Yootil tool library from working.
Then, after refreshing the page, try the Smiley plugin again. After you have tested the plugin don't forget to re-enable Yootil.
If the plugin worked, go through your plugins that require Yootil, disabling each in turn and testing the Smiley plugin as you go along, refreshing the page before you do the test, until you find the plugin that is is interfering with my plugin.
If the plugin DIDN'T work, then you need to do the same thing, but with the plugins that do NOT require Yootil.
If/when you find the culprit, let me know and I'll try to make them play nice together.

In your original post, you said that the Smiley Plugin doesn't work in your game section..
Does it work in other boards on your forum?
Can you also try moving the Smiley plugin to the very TOP of your plugins list.

Thank you for trying. Do you have a test forum? On their forum whenever they insert a smiley on the quick reply box it shows the text but not the animated image. There are no errors in console. It's like that on the default theme/all themes as well. Preview works fine in the full reply. Making a quick reply shows the image and everything works correctly. It's just the preview for the image in quick reply that is only showing as text.

Yup, the mousedown event is missing for the Submit and Preview/BBCode buttons that changes the ::smiley:: code to the [im g] code and vice-versa, I just can't see anything obvious as to why that is.

var messarea = $('.form_post_quick_reply textarea');
var cPos = getSMCaret(messarea);
if(cPos == 0)
{
$(messarea).val(code+$(messarea).val());
var newcPos = code.length;
}
else
{
var mVal1 = $(messarea).val().slice(0,cPos);
var mVal2 = $(messarea).val().slice(cPos);
$(messarea).val(mVal1+code+mVal2);
var newcPos = cPos+code.length;
}
setSMCaretPosition(messarea, newcPos)
}
is adding the url preview
