Well you don't have any descriptions set for any of them. You only have the names. Try fixing that. Then seeing how it works.

I tried deleting the plugin and installing it again, deleting all the data, and I still have this problem. I don't know what might be causing it.

I still can't replicate it. I even just tried on your forum with Chrome and Safari.

That's because it only doesn't work for an admin, members can fill out the template and it works perfectly for them

edit: I've just made my mod an admin to see whether they could replicate the problem, but the template is filled out fine for them too.

Yes, no one else appears to be experiencing the problem.

I never experienced the problem on my forum where I am the main admin.
