I just joined your site and can't see the plugin enabled.
Can you check that you have the latest version. Delete the one you have and download the one above in the first post.
Move the plugin to the top of the list so it runs first.

I just checked this in IE, FF and Chrome and can't replicate the problem (verified by the other comments above, so it may be an old version or you may have some sort of conflict with another plugin)

I use firefox 20.1 and also have IE 8 and have no problems with duplicated icon/buttons
You could try disabling any other plugins temporary to see if any other is conflicting with it
And make sure you have updated browsers

pebble, it might be that one of the other plugins is forcing a call on pagination causing your code (which is listening to pagination) to be ran twice. move the IPlookup code to be the last plugin in the list

This is what I thought but couldn't think of a reason for a plugin to be making a pagination call (and I think it would be against the plugin rules).

All seems Ok for me using firefox 21 - Opera - IE8
You could try disabling any other plugins temporary one by one to see if any other plugin
is conflicting with the IP lookup plugin causing the icons to repeat

I did have that problem when I was using the Awards plugin - I know that plugin has been updated
To fix that issue if have the awards plugin - if that is the problem

My problem was indeed the awards plugin.
Updated it and all is fine now

Uh it is doing x2 buttons with the spoiler tag plug-in installed.
