I am desperately looking for a forwarding application form. One that will allow you to ask 1-10 questions and then have boxes for the applicant to fill in the blanks then when the application is finished and sent it is sent to your websites email or a spot on the admin panel you can view. Any Ideas?

You could use the Custom Post Templates plugin to achieve this. I use it on a few of my forums to work as applications. Plugins can't operate in the admin panel, but you can set up this plugin to work in a Private board ( How to set up a Private Board).

There are 2 "halves" to the Settings tab in the UI. The upper half is where you define the name of your application. The lower half is where you set up questions for it. Here's an example of the upper half - this is for 2 different application, as noted by different names for their Form ID: This is the lower half, where the questions are set up. Note that the Form ID for both questions is the same because they both belong to the same application with the same name in the upper half: When all is done, this is how the posting page will look (this is based on my settings and is an actual application we have on one of my forums): The user then fills in the fields and submits the post as they would any other post. You can use the other tab in the UI to customize how the post will look when posted (it accepts BBCode). Here's what that application's customized post code looks like:

Mallory Faraday, as a ProBoards' employee I was able to look at your plugin settings, and for the Custom Post Template plugin I don't see any information entered under the Settings tab.

Mallory Faraday , if you would like to re-enter the information, I'd then be happy to take a look and see why it's not working for you. The plugin does work. We are using it here on this forum for our Affiliate Exchange board. You can go this board and click create thread to see the template we set up.

Mallory Faraday, I currently use the plugin on one of my forums and it works without issue. I'm very familiar with the plugin , so I can help you with any questions you may have. While I can't access your settings like Scott can (because I'm not a PB employee), I can look at the source on your forum to see if I see any issues with your settings. All I'd need would be your forum URL (you can PM it to me if you don't want to post it) and for it to not have Guests Must Login enabled. I don't need to post anything or access any boards. Up to you how you want to proceed.
