So this isn't a new plugin and may not have all the features you want, but since you've been bumping for a while I thought I'd pass this along in case it fit your needs enough even for a temp :3

Custom Post Template Plugin

I'm going to go down your list of Features in the first post:

- You can set BBCode so you will be able to design it however you want. While the plugin itself doesn't give CSS access (I don't think..?), it's easy enough to work around that with the Style Tag Plugin which would give class styling capabilities to your BBCode.

- You can setup whatever text fields you want, editable by members

- I don't believe you can restrict it based on groups (though there is a group option for the fields in the form), but you select the Boards which it should affect (you can setup multiple forms and have them affect different boards).
    *Extra note - I'm just referring to this plugins offered options & not the general ability to set a plugin's usability based on member groups. So MSG's note could work for you!

- You choose when this form pops up (New Thread, New Reply, or Both), and you can choose whether any fields are mandatory or not.

- The ability to choose where/how an inserted link goes/looks already falls under the first point.

- Since you can set widths/heights (or max/min height/width as you desire) in BBCode the ability to assign image heights falls under the first point. Google CSS max-height (or width, or min-height, etc) if you're unfamiliar with these properties.

- There is no history capability

Just to note, you can code it to restrict based on a member's group - I've done that with my Navbar Links plugin. In my plugin, if no group is added, it defaults to everyone who can see the link. If at least 1 member group is added, the link will only show up if the member is in that group. If you want to know more, feel free to PM me.

Unfortunately, that plugin doesn't fit the needs of my forum - I've also tried it several times in the past and never like the result, no matter how much fiddling I do. Thank you for your response, though! I appreciate the effort.
