Unfortunately, you can't. The best you could do would be to turn Restricted Registration on and request that information. You can then reject anyone who doesn't provide the answers you seek / any answers at all. If you want to do that, head over to your admin panel under settings > login and registration, and set Restricted Registration to "yes". Once you do that, you can enter a message for registrants containing your questions. From there, anyone who attempts to register will find that they need to add in a message for you with their registration. You'll receive a message in the admin panel when you have new registrations to approve, and you can accept / decline as needed from there. Hope this helps! I was helped with this recently, so now I'm able to pay it back to help others. My forum is private, and we wanted to know the full names of new registrants, how they found out about us, and one other thing, before they got full access to the forum.
So, I set it up to put new registrants into a Provisional Member group that allowed them to see only one category and one thread. That thread contains only one post, an explanation that once they message staff with the answers to our questions they will be removed from the Provisional group and placed into the default new member status. I placed a link in the post that automatically created the message and populated the Recipients with our staff.
Here is the way to set it up. This is not original from me. I found it elsewhere here, but forget where:
Obviously, you'll want to edit the questions the new member has to answer.
This works perfectly for what I needed.

I don't have a Create Category Button . What am I missing?

What am I missing? I think you're misreading that part of the instructions. The create category button is the end part of step 4 not the beginning of step 5.

That looks like you are editing a category, and not creating a new one. Click the Create New Category button which you should be able to see at the top left, and it will allow you to create a new category.
