This looks to simply be a custom page that uses the HTML widget to embed a google document.

To create a custom page, go to admin > structure > custom page. Select the page layout you want, and add the HTML widget.

Then, create a google document and follow the below instructions to create an embed code to place in the HTML widget:

Open a file in Google Docs, Sheets, or Slides.
At the top, click File and then Publish to the web.
In the window that appears, click Embed.

Click Publish.
Copy the code in the text box and paste it into your site or blog.
