A banned user list?

Could someone help me make something like that?
It shows all banned users, the date of the ban, and the reason of banning.

If you cant help, could you tell me at least where to enter code at?

1. This is the wrong place for this. Always check the board descriptions/stickies first

2. The easiest way would be to just get your Mods to post in a special "Banned Members" board when they ban someone.
