What specifically are you asking for? The design you linked seems fairly straightforward, try using the Colors & Theme Creator to reproduce it (Admin Area > Themes > Colors & Theme Creator). Then go to the Plugin Database here on support and look up a Sidebar plugin and an affiliate table plugin. There are tons of resources here for you already, and doing it yourself will provide valuable experience.

We highly recommend using a desktop computer for the initial setup of a forum. The larger screen and full keyboard make setting up a theme and plugins the way you want it much easier.
As for a banner, if you have an image you want to use, simply go to the Colors & Theme Creator, and towards the bottom of the page there's a field for "Banner Image", where you can upload an image, link an image, or choose from a set of default images.
