Character Log Code

I'd love for someone to make me a code for my character log for a role-play site. 

I'd love for it to be creative and flow in alignment.  I want there to be a place for a picture. Thanks.

I'm not a role-player so I have no idea what a character log might look like. If you know of a site that has one, linking to it would be very helpful. If not can you give me a lot more detail. Also would this be in a post or on a blank page via your headers/footers? Lastly, is your site a V5 site or V4.5? (Why a link is always nice.)

A character log is where you have a thread that keeps track of all your characters. The site is v.5

I understand it is a log of your characters but why do you need a code? Can't you just list them? Or are you wanting sections with different aspects of the character? You can use the table icon to make columns and rows and use some css.  Let's see if I can do a sample and you can tell me if I'm way off base or not.
