Is there any clean way to add a page to the navbar? Or does this still need to be done the v4 way, by adding a link, e.g. path/to/index?page=mynewpage, and then just replacing all of the page contents with the new content with JS?
EDIT: I see this can be done through the structures area, but not the plugin system. When relying on this with a plugin, should the plugin instructions just include instructions to manually create a page?

if you want a clean page, I would have them create the custom page themselves so you can edit it. otherwise, yes, its removing elements from an existing page and putting in your own. Many of my plugins that added tabs to the profile screen simply added a new tab to click which put a keyword in the url. then the plugin looks for that word in the location check and strips out the contents of the page I dont want (usually just the div.content stuff). the tabs would remain then.
