Seeking a code

I'm not sure exactly what it's called that I'm looking for but I'll try to explain it the best I can so hopefully it's not too confusing

The code I'm looking for would be put into the specific forum it is wanting to be used for - I know on one of my old RPs we did this for our character applications. Instead of the traditional copy/paste option for the applications instead you just create a new topic and instead of the open/free-form way to create a new post instead you are presented with questions that you answer in a field then when you post it all the coding is done for you. Less chance of accidentally doing something wrong like with the copy/paste option.

I could've sworn we already had a Plugin for this, but I was unable to find it. Assuming you are using V5, you'll want to Request a Plugin =)
