Looking for a plugin

Looking for a way to add a drop down with a form for our members to fill out.  In this case it will be a penalty appeal process.    The form needs to have a place for the members name, date , reason for the appeal, and a way to attach a file, and a button to submit that would send this to the admins email.  Is there something out there they would work that I have missed?  Thanks in advance for any help.

Where would I find this to check it out?   I tried a search and found nothing.

ok thanks.  Found it and will have to research to see if its what Im looking for.  It just needs to be a simple form for our members to fill out.

It was based off the v4 version of the code, and that is what people used to use all the time
