Custom Post Template with Styling

Im wondering if one of you very talented template guru's can assist me with creating the following ....

Basically i want to re-construct the Thread creation page to display a custom template when creating new threads but only on specific boards.

They would include the following fields ...


Subject - Thread Subject field

(Section 1)

Cover Image URL - Auto Embeds an image in the threads post. (Field that auto appends an image)
Tagline - Standard text field input
Genre
- Standard text field input
Release Date - Drop down = Day, Month and Year 
Storyline - 5 - 10 line text area box


(Section 2)

Cast Overview (Header) - Horizontal / Break line underneath this header

Actor/Actress - Left side text box 
Character Portrayed - Right side text box

Horizontal / Break line here

Information courtesy of IMDB.com. - Complimentary Italic text offset to the right



Personal Review/Opinion/Thoughts - Larger text area
