You are leaving out information that is required. The rules for Graphic Requests asks for a link to your forum and it must be open to guests. If there are any images you want used, you should go find them yourself and make sure they are free to use (not copyrighted). Any specific colors you want your banner and any text on the banner? If text on the banner, what should the text say and what color and any particular style?
