I am making a gaming community forum and I want there to be a page where users ranked of the forum owners choosing can view the username of the user banned, the reason for the ban, the duration of the ban, status of the ban, and who issued the ban.

A plugin can't pull information that isn't already available on the page. You can already use a combination of Security > Security Log and Members > Ban Members in the admin area for this anyways.
In Members > Ban Members a ban reason can be specified in the Notes column. This field is usually available to fill out in the window that comes up when banning a member from any page on the forum.
In Security > Security Log click on the All Logs dropdown and select Admin Logs > Bans to see every ban-related action on the forum as well as which user performed it.
