Looking for a board remodel

Hello there!
I've been working on a new RP forum, and have hit a bit of a snag. I was hoping someone could help me out by 'coding' up a board template remodel for me? I used to be rather decent at it, but (after my 3 year break) I guess i'm more than a bit rusty - because I haven't been able to make anything work and haven't found anything on any of the resource sites either.
Anyway, something like THIS is what i'm looking for (I through it together real quick in gimp).
It looks pretty simple, so i'm hoping that someone will be willing to help me out here.
