Re-creating a forum header!

My friend and I are re-creating an old forum we lost access to, and we were wondering if anyone could help us with re-creating this header.

We honestly have no idea how we made this before..what plugins if any were used--and we'd love some support. Basically just the section titled "WWE '12 Main Menu. Press Start."
