you're making something and accidentally press ctrl+w and then press no when it asks you if you want to save in stead of cancel? just happened to me and i hate it >

Haven't done that exact thing yet. But I have finished something, flattened it, saved it, then closed up Photoshop only to remember I still had to add something to it but now can't because I had flattened it. >.>

No, but I use the New layer shortcut(Ctrl+Shift+N) and I accidentally hit Ctrl+Shift+M. :/ I have to wait for Imageready to open, then close it. Which takes very long for me.
(Ctrl+Shift+M is Merge to Imageready.)

I did the closing thing once - I can never really be bothered to read those warning messages so I guess I deserve it lol. I don't tend to use ctrl+shift+n although if I remember to then I will so I've never really done the imageready thing.
