when you edit a post and enter an Edit Reason, then go back and edit that post again, it looks like the Edit Reason disappears. This is potentially very bad for my forum, which uses Edit Reason to include staff notes when monetary transactions have been accomplished. Is this a bug? Am I misunderstanding?

I would consider this a bug. It should either add to the previous reason or load the reason to be edited.

As far as I remember it's always been like that (V5). There are some information posts I have to edit almost monthly and the reason for editing box is always blank on subsequent edits.

Not a bug.
The edit reason is simply an optional field to provide users with a simply explanation of why a post was edited. It's not meant to be a log of any sort. So when a post is edited again, the next person to edit it has the option of supplying their own reason.
A plugin could easily be made to keep a history of a post's edit reasons, though.

Dang, there's a plugin for this called Admin Note that I was originally using for this purpose but then I decided to use my own coding in the layout templates instead to reduce plugin space, but I'll just go back to using Admin Note then. Thanks anyway, guys.
